Refer someone to us
Social workers, discharge planners, service coordinators, clergy and health professionals can refer a person to us. The person or family can also call us directly.

Include the person's name, phone number and county, and their health plan or Medicaid number if you know it. Please send health information by phone or fax only, not by text.
Who we serve
Services for people of all ages who have difficulty with activities of daily living, whether or not they currently have Medicaid.

We offer the agency option only (not Consumer Directed Services).
Contracted with
HHSC, TMHP (PCS), Superior, Driscoll, United, Molina and Wellpoint. For people without Medicaid or a health plan, we offer private pay. How Medicaid pays for home care
